Life Insurance & Long-Term Disability
Full time salaried employees are eligible to purchase voluntary life & long term disability insurance, subject to all terms and conditions of the agreement between QualiTest and the insurance carrier, Colonial Life.
All eligible employees may begin coverage on the first of the month following 30 days of employment.

As an employee of QualiTest, you have the opportunity to apply for personal insurance products from Colonial Life for you and your family! These benefits can enhance your current benefits portfolio and can be customized to fit your individual needs.
Disability Insurance – Replaces a portion of your income to help make ends meet if you are totally disabled due to a covered accident or covered sickness. This program also has a post-delivery maternity benefit. Can replace up to 60% of your salary to a maximum of $6,500/month.
Life Insurance –helps provide financial security for your family members. Term, whole and Universal policies are available. Policies are also available for spouses and children.
Accident Insurance – Accident Prone? Weekend warrior? Skier? Cyclist? This coverage helps offset unexpected medical expenses, such as deductibles and co-payments that can result from a fracture, dislocation or other covered accidental injury including sports injuries, car accidents, falls and spills, etc. This program covers both on and off job accidents and includes worldwide coverage. Spouse Disability coverage is also available through this program.
Cancer Insurance –The risk of developing cancer is very real. In the U.S., men have a 1 in 2 lifetime risk and for women the risk is 1 in 3. This policy helps offset the out-of-pocket medical and nonmedical expenses related to cancer that most medical plans may not cover. This coverage also provides benefits for specified cancer-screening tests.
Critical Illness Insurance –This policy complements your major medical coverage by providing up to $75,000 in a lump-sum benefit you can use to help pay the direct and indirect costs related to a covered critical illness, which can often be expensive and lengthy.
